Seems the pinholes have to start from 0 for PPTP-GRE to work all is now working.

Thanks Mark for the tip. So should be

pptpd-47 192.168.10.1 0 0 65535 PPTP-GRE
pptp 192.168.10.1 1723 1723 1 TCP
smtp 192.168.10.1 25 25 1 TCP
